Begining January 1, 2020 Prudential will be AHA's new carrier for Life Insurance, Accidental Death and Dismemberment. The AHA provides benefit eligible employees with Life and Accidental Death and Dismemberment (AD&D) insurance in the amount of two times your annual base pay with a maximum beneﬁt of $1 million (AD&D matches the Basic Life benefit).

You may choose to purchase additional life and AD&D coverage for yourself and your family members. If you select coverage above Guaranteed Issue, you may be required to complete Evidence of Insurability paperwork. You pay the full cost of coverage, which is available in the following amounts:
